To On-Board with Elite Care Management, you will need the following qualifying credentials:

• Driver’s License
• Social Security Card
• Auto Insurance Card
• Indiana CNA or HHA certificate or Illinois CNA certificate
• Current CPR Card
• 2- step TB test reading, Chest X-Ray, or QuantiFERON Gold Blood Test – within last 12 months
