Elite Care Management is seeking an experienced and motivated Registered Nurse (RN) Clinical Manager to lead and support our growing home care team. This leadership role is responsible for overseeing clinical operations, ensuring quality patient care, supporting field staff, and maintaining compliance with state regulations.
Our ideal candidate is a strong communicator, organized leader, and passionate advocate for high-quality care delivered in the comfort of our clients’ homes.
Responsibilities
- Provide clinical oversight and supervision of nurses and caregivers in the field
- Ensure care plans are implemented and updated appropriately
- Conduct client assessments and reassessments as needed
- Collaborate with families, physicians, and interdisciplinary teams
- Provide guidance and clinical support to staff
- Assist with scheduling and staffing as needed
- Ensure compliance with state regulations, agency policies, and best practices
- Participate in quality assurance and clinical documentation review
- Support training and orientation of new clinical staff
Qualifications
- Current Registered Nurse (RN) license in Illinois
- Minimum 2–3 years of nursing experience, preferably in home care, hospice, or private duty
- Previous supervisory or management experience preferred
- Strong organizational, communication, and leadership skills
- Ability to manage multiple priorities in a fast-paced environment
- Knowledge of home care regulations and documentation standards preferred
- Valid driver’s license and reliable transportation
