Elite Care Management can provide services from 6 hours to 24 hours of RN/LPN and caregivers. At this time, we develop the Plan of Care with the patient, physicians, discharge planners, case managers and family. Our staffing coordinators carefully select the caregiver with the necessary experience that is best suited to meet the needs of each of our patients.
Elite Care Management begins by reviewing your needs through discussions with yourself, family members, and various healthcare providers. At this time we develop the care plan and coordinate your care with the patient, physicians, discharge planners, case managers and family. Our staffing coordinators carefully select the caregiver with the necessary experience that is best suited to the needs of each of our patients. Next, we arrange for the caregiver to visit so you can participate in the selection process. After services begin, our clinical managers and staffing coordinators will continue to implement our system of ongoing personalized contact to ensure the best possible care.
Elite Care Management can provide services for as long as the patient, family, and/or case manager deems necessary. Private duty services cannot receive payment from Medicare. Instead these services are reimbursed through private pay, private insurance, long-term care insurance or workers compensation.
