Job description:
Elite Care at Home specializes in delivering exceptional visit-based healthcare services directly to the homes of our patients. We are seeking a highly organized and detail-oriented Administrative Assistant / Home Health Coordinator to support daily operations in a fast-paced home health agency. This position plays a critical role in coordinating patient care, managing clinician schedules, and ensuring documentation and regulatory compliance are maintained.
Responsibilities:
- Coordinate new patient referrals and schedule Start of Care visits
- Manage daily clinician schedules (RN, LPN, CNA, Therapists)
- Monitor coverage gaps and adjust staffing as needed
- Track physician orders and ensure timely signatures
- Maintain accurate records in EMR (Axxess or similar system preferred)
- Assist with insurance verifications and authorizations
- Support EVV compliance and documentation deadlines
- Communicate with patients, families, and referral sources
- Assist with QAPI reporting and compliance tracking
Qualifications:
- 2 years of healthcare or home health experience preferred
- Experience with EMR systems (Axxess preferred)
- Strong administrative, scheduling and coordination skills
- Knowledge of insurance verification and authorizations
- Excellent communication and customer service skills
- Proficient in Microsoft Office
