As a Skilled Nurse (RN or LPN), you’ll provide one-on-one care in a home setting—allowing you to build meaningful relationships with patients and families while delivering focused, uninterrupted care.

Key Responsibilities:

• Monitor patient condition and report changes to supervisor or physician
• Follow all nursing policies, procedures, and care standards
• Communicate clearly and professionally with patients, families, and team members
• Provide care based on individualized care plans and update as needed
• Administer medications and treatments, evaluating effectiveness
• Respond promptly to changes in patient condition

Qualifications:

• Active Indiana RN or LPN License
• Current CPR Certification
• TB Clearance (one of the following): 2-step TB test, Chest X-ray (within 12 months), QuantiFERON Gold (within 5 years)
