Role Description
We are seeking a Full-Time Home Health RN to join our Naperville-based team. In this role, you will provide skilled nursing visits to patients in their homes. Responsibilities include completing assessments, documenting OASIS when applicable, administering medications, coordinating with the care team, and educating patients and families on individualized care plans. Flexible scheduling is available.
What You’ll Need to Join:
- Illinois Registered Nurse (RN) license
- Home health experience with strong OASIS documentation skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaborate with the care team
- Knowledge of home health procedures and medical equipment
- Compassionate, patient-centered approach
- Strong organizational and time-management skills
- Valid driver’s license and reliable transportation
- BSN or related degree is a plus
