Shifts Available:

• 8am-11am Mondays, Wednesdays, and Fridays
• PRN – Various days and times

Elite Care Management has over 25 years of experience in home care. When you join Elite you will work in a team-oriented culture and make a difference every day in our patients’ lives!

Responsibilities may include the following:

• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor or case manager.
• Support mobility by assisting patients with lifting and transferring as needed.
• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and eating.
• Light house-keeping duties, such as changing bed linens, laundry, cleaning, or preparing meals.
• Enhancing quality of life and building relationships with patients through conversation, doing activities together, and finding stimulation to keep them mentally healthy and alert.
• Monitor patients’ pulse, temperature, and respiration.

To On-Board with Elite Care Management, you will need the following qualifying credentials:

• FL State Certification – CNA or HHA 75 hour training program
• Current CPR Certification
• Social Security Card
• Driver’s License and Auto Insurance Card
• TB Test (2-step within the last 12 months OR QuantiFERON within 5 years)
