Role Description
- Full-time or part-time on-site role
- Providing compassionate and personalized care to patients in their homes.
- Day-to-day tasks: conducting assessments, administering medications, coordinating with other healthcare professionals, and educating patients and their families on care plans
Qualifications
- Illinois Registered Nurse (RN) license
- Experience in home health care or a similar setting & Oasis Documentation
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of medical equipment and procedures
- Compassionate and empathetic demeanor
- Attention to detail and organizational skills
- Valid driver’s license and reliable transportation
- BSN or related healthcare degree is a plus
